• The network includes more than 4,000 physicians and covers an 11-county area in southeastern Wisconsin, including: Dodge, Jefferson, Kenosha, Milwaukee, Ozaukee, Racine, Rock, Sheboygan, Walworth, Washington, and Waukesha Counties
• When traveling outside the 11 counties you have access to Humana’s national network
