Where to File a Claim
Retiree Death Reports should be referred to Benefit Express at 888-409-3300. Subsequently, you or your beneficiary will be provided instructions for filing a claim.

How To File A Claim
Please follow the instructions, complete the forms and provide any requested documents.

If a Claim Is Denied
If a claim for life insurance or accidental death and personal loss insurance is denied, in whole or in part, a notice will be sent by the Insurance Company explaining the reason for the denial including references to the appropriate plan provisions. TOP

If you have questions or do not agree with the denial, written inquiry must be made within sixty (60) days from the date the claim was sent to the DaimlerChrysler Corporation Group Insurance Office at the address indicated in the Administrative Information of the summary plan description.

  • The letter should state the reason the claim should not have been denied and should include a copy of the notice and any additional documents, records, information, or comments that have a bearing on the claim.
  • Written notice of the final determination will be sent within sixty (60) days after receipt of the appeal. If special circumstances require an extension of time, you will be notified. TOP

If a claim is denied in whole or in part, a notice will be sent by the Insurance Company explaining the reason for the denial, including references to the appropriate plan provisions.

If you have questions or do not agree with the denial, written inquiry should be made within sixty (60) days from the date the claim was denied to the Insurance Company office that denied the claim, furnishing all information supporting the appeal.

  • The appeal will be reviewed by the Insurance Company and written notice of the final determination will be sent normally within sixty (60) days after receipt of the appeal.
  • If special circumstances require an extension of time, a notice of such extension will be sent within sixty (60) days following the appeal. TOP

In the event of a spouse or dependent child death claim, the Insurance Company should be contacted at 1-800-377-3293. Claims should be filed promptly.

You or the applicable beneficiary will be provided forms and instructions for filing a claim.

If a Claim Is Denied

If you wish to appeal the denial, written inquiry must be made within sixty (60) days from the date the claim was denied to the Insurance Company office that denied the claim, furnishing all information supporting the appeal. In the event of your death, your local DaimlerChrysler Corporation Personnel Department should be contacted. To report the death of your spouse or dependent child, contact Benefit Express at 888-456-7800. You or your beneficiary will be provided forms and instructions for filing a claim. TOP

To report a dismemberment loss claim on yourself, your spouse, or your dependent child, please contact the insurance company.
