Voluntary Long-Term Care Insurance (Hourly, CET)

What is it?
The Long-Term Care Insurance Plan is a NEW, voluntary, employee/participant-paid program negotiated between the UAW and DaimlerChrysler and offered through John Hancock Life Insurance Company. Through this program, you can purchase valuable long-term care insurance to help defray the costs of long-term care that you or a family member might need. Long-term care insurance can cover care in your home, a qualified adult day care center, an alternate care facility, or a nursing home. And unlike government programs, long-term care insurance allows you to receive the type of care you need in the setting that's right for you.

Who is eligible?
The plan is available to full-time and part-time, actively-at-work, union-represented employees working at least 80 hours per month on U.S. payroll. It is also available to employees' spouses/qualified same-sex domestic partners; and their parents, parents-in-law, grandparents, grandparents-in-law, siblings and adult children residing in the U.S. (Spouses/qualified same-sex domestic partners, siblings and adult children must be age 18 or older to be eligible to apply.)

When can I enroll?
From October 18 - November 19, 2004, eligible, actively-at-work employees will receive guaranteed acceptance into the plan. Guaranteed acceptance means that you will be automatically accepted into the plan without having to provide proof of good health.

After November 19, 2004, you may still apply for coverage but will need to provide proof of good health before being accepted into the plan. Your eligible family members (as described above) will need to provide proof of good health when applying at any time.

When will the plan be effective?
The plan will be effective December 1, 2004 when you enroll during the October 18 - November 9, 2004 enrollment period.
